Welcome to CVMA® NC 15-1, The All-American Chapter.  This is the New Member Portal and here you will find all the documents and processes needed to complete your application to join.  Please read this entire page before continuing.  If you have any questions, please email the Chapter Secretary or speak to your Mentor.

The application process is as follows:

Step 1: Application  Please fill out the Member Application and Patch Agreement by downloading them to your computer.

Do not hand write except for the initials and signatures. We do NOT accept PDF filled initials and signatures.

INITIALS AND SIGNATURES MUST BE HANDWRITTEN

For the Patch Agreement, select
$49.99 on the 4th initialed line for Full Member (12.5 Inch) and $39.99 for Full Member (10 Inch), Support, and Auxiliary Members.

Please ensure both the Application and the Patch Agreement are
signed and dated
Other forms you will need to send/upload are: Copy of your DD Form 214 - Member 4 (Retired or Discharged Members) or Soldier Talent Profile (Active Members; Please print out the additional rows of assignments showing combat deployments) (For Full or Support Applicants), Motorcycle Insurance (For Full or Support Applicants), Current Motorcycle Registration (For Full or Support Applicants), Driver’s License w/ Motorcycle Endorsement (For Full or Support Applicants) and Marriage Certificate (For Auxiliary Applicants).

Ensure you black out your SSN/DODID on any forms.  Only your Full Name must be visible on the forms.

Naming Convention for Files (
save all in .pdf format)
Member Application: Smith_Application
Patch Agreement: Smith_Patch_Agreement
DD214/ERB/ORB: Smith_DD214 (or STP) - please use long form (Must include characterization of discharge)
Marriage Certificate: Smith_Marriage_Certificate (For Aux Membership)
Motorcycle License: Smith_License
Motorcycle Insurance: Smith_Insurance
Motorcycle Registration: Smith_Registration
Once completed, email all forms to the Chapter Secretary or upload all forms to the appropriate boxes below.

Step 3: Pay your Application Fees (please email sec@cvma15-1.net when complete) Before your Application can be submitted to the National Board, your Application fees must be paid in full using PayPal.  PayPal allows you to use your account or to enter any credit card as a guest.  To pay your Application Fees, use the form below or pay using your PayPal account to: treas.nccvma15.1@gmail.com.

* If you are applying as a Full Member and transferring to another Chapter upon immediate approval of your application, then only pay the Annual CVMA® National Dues of $20.00. You will pay your backpatch fee and Chapter Dues at the gaining Chapter.

Step 4: Order your Vest Patches  Once your application has been fully processed and accepted, you will receive an email with your Member Number and a link to purchase the Vest Patches for Induction. Your American Flag, "Skully", and "Scroll" can be purchase via the Chapter Store. Nametags (for those of you who have a road name) can be purchased through the National Store. See the new member email for details. Once your Back Patch is received and you have purchased your Vest Patches, your Mentor will assist you in getting these items sewn on your vest.  Once this is done, your induction will be scheduled.

Please email the Chapter Secretary or your Mentor if you have any questions or need assistance completing your Application.
Step 2A: Safety Inspection (if applicable)  The Sergeant at Arms (SAA) or the Chapter Road Captain (CRC) will coordinate with you to conduct a safety inspection of your motorcycle. This will include a T-CLOCS Inspection and a review of your current driver’s license with motorcycle endorsement, motorcycle registration, and insurance.  This is normally done at a Chapter Meeting.  Once complete the SAA or CRC will inform the Secretary.

Step 2B: Mentor Assignment/Attendance Requirement  For your application to be submitted to the National Board, you must first complete all previous steps (above) and have attended (3) chapter calendar events, one of which is a chapter meeting. You will be assigned a Mentor by the SAA/CRC who will assist you in becoming familiar with the National and Chapter Policies & Processes. During this time you will introduce yourself to the members of our chapter as well as get to know your reding style. Once complete, the SAA/ CRC will inform the Secretary. Your application will move to Step 3.
